We now offer the convenience of completing the Christian Formation Registration form electronically.  If payment is required, you may drop your money off at the parish center, mail it in, or put it in the Sunday collection.

DIRECTIONS FOR COMPLETING THE FORM:  Each question in black shading represents a section on the form.  There are sub-questions underneath the main question.  Please answer all questions and be as complete as possible.  Please contact us if you have any questions or experience any difficulties.  Also - be sure to follow the directions at the end of the form - it's very important.
